The Arizona Department of Environmental Quality asked the Arizona Department of Health Services to evaluate the health threat to residents from exposures to ash fallout from a fire at an asphalt plant located on the Salt River Indian Reservation in Mesa, Arizona. The facility has recently been bought by Vulcan, Inc. The site is located just north of the Salt River between Dobson and Alma School Roads. April 13, 1999.

A resident of Wickenburg, Arizona and her husband reported experiencing some adverse health effects that she felt may be attributed to a recent pesticide application to her home. The family requested the Arizona Department of Health Services to evaluate whether the remaining levels of pesticides present in her home would cause any health effects. The resident reported to ADHS that their symptoms continued to worsen over time, and that the chemical continued to pool in their house even after being wiped up several times, so that she and her husband vacated the house. The couple underwent extensive testing, and are receiving oxygen treatment to alleviate their symptoms. No medical records have been submitted to ADHS for review, and it is difficult to conclusively analyze health outcome information as it has been self reported.

This report evaluates the potential exposure pathways associated with vermiculite concentrate processing activities at the Ari-Zonolite facility. The site is located in the near downtown area of Glendale, approximately ½ mile from the city hall, downtown shops, and other buildings. The former Ari-Zonolite facility received vermiculite from the Libby, Montana, mine. From 1951 to 1964, the site was leased the site to the Ari-Zonolite Company. Following the removal of the vermiculite concentrate processing equipment in 1964, several businesses have occupied the site. None of these businesses were involved in vermiculite processing activities. The last occupant of the former vermiculite processing building was an automotive restoration business, which vacated the site in 2002.

The objective of this assessment is to evaluate whether a public health hazard exists as a result of environmental exposures in Hayden and Winkelman, Arizona, located near a smelter operated by ASARCO, Inc. Some of the residents of these towns have expressed concerns that metals from smelter emissions over the years have been causing health problems. This report uses environmental data collected from water, soil, and air in the Hayden and Winkelman area to estimate environmental exposures. The study also evaluates available health outcome data from previous epidemiological and biological-monitoring studies conducted in the area. The report concludes that exposure to sulfur dioxide occasionally poses a short-term public health hazard to sensitive asthmatics. These episodes of higher levels of sulfur dioxide occur infrequently. They do not appear to pose a health hazard to persons without sensitive airways or asthma. Other environmental exposures do not appear to pose a public health hazard.

The Blue Ridge Elementary School began receiving complaints from parents and staff in 1996. The primary complaints were related to air quality problems associated with renovation activities. The school responded to these complaints by hiring a consultant to evaluate the environmental conditions at the school. The school received a report

The Blue Ridge Elementary School began receiving complaints from parents and staff in 1996. The primary complaints were related to air quality problems associated with renovation activities. The school responded to these complaints by hiring a consultant to evaluate the environmental conditions at the school. The school received a report in 1997 containing several recommendations that the school implemented in 1997. A group of parents continued to express concerns about environmental conditions at the school. This group suggested that environmental exposures might be causing some students to be absent from school. In response to these complaints, the school hired another environmental consultant to conduct a follow up environmental assessment in December 2002. This report summarizes and evaluates the environmental data collected for the school and examines school attendance rates to determine whether students are absent more than at similar schools and if so, whether environmental conditions could be a contributing factor.

The purpose of this report is to document the evaluation of the public health effects of the air emissions from the Central Garden and Pet Supply Warehouse fire that was verbally provided during the response efforts to the fire event. The facility contained a variety of lawn and garden supplies including pesticides, herbicides, and pool maintenance chemicals. The fire also involved a portion of a pharmaceutical supply firm which contained medical supplies ranging from liquid and pill-form medicines to sterile syringes and other physical supplies.

The purpose of the report is to evaluate environmental conditions and data at the site to determine whether a public health hazard exists as a result of exposure to contaminants in soils at the property. The City of Phoenix considers the site to be one of its highest priority brownfield projects due to its size, location, and threat to the public. Nearly the entire site is covered with solid waste including metal debris, tires, empty drums, construction debris, refuse, ceramics, empty alcoholic beverage bottles and cans, and human waste. A large pile of automobile fragments known as auto “fluff” is present on the southeast corner of the property. Contamination of soils at the site has occurred by disposal of auto fluff and transformer oils. The contaminants at the site include various metals and polychlorinated biphenyls (PCBs).

The objective of this health assessment is to provide an evaluation of current and future potential health risks that may result from exposure to residual organochlorine pesticides at Franklin Elementary School in Phoenix, Arizona. The school has been unoccupied for the past 10 years. The historic school building is being restored by the school district, which plans to reopen the building for use as an elementary school. Several organochlorine pesticides were discovered in the soil during routine environmental sampling done in conjunction with the renovation. In particular, chlordane and dieldrin, banned in the United States in the early 1980's, were present in concentrations above screening levels.

In 1964 W.R. Grace purchased the company that had previously occupied the site and, following the relocation of its vermiculite exfoliation furnace

The W.R. Grace facility in Phoenix, Arizona, received vermiculite concentrate from the Libby, Montana, vermiculite mine. W.R. Grace Company has owned and operated the Arizona site since 1964. In 1964 W.R. Grace purchased the company that had previously occupied the site and, following the relocation of its vermiculite exfoliation furnace from Glendale, Arizona, began processing vermiculite concentrate and marketing it under the Zonolite® brand. The objective of this health consultation is to evaluate exposure pathways and potential health effects in those persons who, between 1964 and 2002, may have been exposed to Libby asbestos as a result of vermiculite concentrate processing activities and waste materials from the W.R. Grace exfoliation facility in Phoenix.

The objective of this health assessment is to provide an evaluation of the health risks that may result from exposure to abandoned mine tailings in Klondyke, Arizona. The tailings are remnants from an earlier flotation mill and mining operation that was in business from the early 1900's until 1950. Environmental data indicate that the 70,000 cubic yards of tailings that remain on the property have contaminated the soil and nearby creeks with heavy metals. This report evaluates the potential pathways of human exposure to the contaminated media. The contaminants of concern further evaluated are lead, arsenic, cadmium, mercury, manganese, and boron. Child health issues and community concerns are also an integral part of the evaluation.
